What to expect during titration

GI side effects are common

Nausea affects ~44% of semaglutide and ~33% of tirzepatide users. It's usually worst in the first 1-2 weeks after each dose increase and improves.

The hardest dose jump

For semaglutide, the 0.5 mg to 1.0 mg step is often the toughest (it's a 100% increase). For tirzepatide, the 5 mg to 7.5 mg step tends to spike side effects.

Eat smaller meals

These medications slow gastric emptying. Eating pre-medication portion sizes is a top cause of nausea and bloating. Avoid high-fat, greasy foods after dose increases.

Slowing down is fine

If a dose increase causes persistent nausea beyond 2 weeks, drop back to the previous dose for a few weeks and retry. Many providers use 6-8 week intervals instead of 4.
